The Immune System, the Brain, Ability and Mental Illness

​  This is the 50,000 feet overview-- like a lot of what we do.

              There is a lot going on with mental health problems, they often involve problems with:
  • the Gut
  • the Immune System
  • the Brain
  • Epigenetic predispositions to changes to the Immune System
  • Other Genetic Factors
  • Etc.

                 That's A LOT -- and it's probably not all of it.  That is why our current systems are so challenged and why we have a "Criminal Justice Crisis" and a "Mental Health" crisis -- and we have an ETHICAL CRISIS in the Mental Health Profession and the Criminal Justice System that ripples throughout the Society.

                   Unlike Victor Hugo's 19th Century France, it's been awhile since modern prosecutors were interested in prosecuting people for stealing a loaf of bread to feed their families, but I'm sure there are exceptions.

                      We often have pretty fuzzy ideas about how the:
  • actions,
  • artistic endeavors or
  • business schemes (legal or illegal)
of people with significant emotional dysregulation are connected to the brain and the immune system.

                      AND let's be clear -- our ideas aren't CRYSTAL CLEAR -- and that is part of the point.   

                       BUT there is scientific backing for connecting:
  • the immune system to mental illness
  • the immune system to social behavior
  • the immune system to positive benefits of music
  • professional musicians having a high rate of mental illness 
  • entrepreneurship being associated with some forms of mental illness 
  • etc.

                         This gets back to our LIFE-SAVING & SOCIETY-SAVING POINT -- a Strength-Based Approach is NOT just something nice to do.

                          People are contending with COMPLEX BIOLOGIES that even in 2020 are only fractionally understood.

                            We've developed some coping skills over the past few thousand years that aren't perfect BUT often help.

                            Every person in this society needs a PATH & SUPPORT to EXERCISE THEIR STRENGTHS -- not just as something nice to do, but for their survival and ours.

What's The Point?  Many people with emotional dysregulation have profound abilities (not perfect abilities) and they need to exercise those abilities not just because they want to (although they do want to).

Exercise of those abilities often helps them function (although not perfectly).  

And for many of us it is as ESSENTIAL as WATER.

What's The Point?   The Point is that much of our current system of justice is as Cruel as Victor Hugo's "Les Miserable" in 19th Century France -- given current knowledge and knowledge of lack of knowledge.

The Point is NOT that people with behavioral issues and bad intent may not present a SAFETY THREAT -- but much of what we do EXACERBATES that.

​ We need Federal and State governments to obtain independent analysis of the ECONOMICS of rapidly bringing Mental Health Clinical Practice in line with current research, especially regarding abnormalities in the Immune System.
               The HARD TRUTH is we have a Mental Health Crisis in large part because our treatments aren't very good.
                If we really want to TURN THIS AROUND, that is going to require new understandings of "Mental Illness" -- which we are beginning to have.
                 Unfortunately, we have a very POOR INFRASTRUCTURE to get these new understandings to Clinicians in a TIMELY MANNER.

       I remember once hearing Political Columnist & Commentator George Will say it must have been so COMFORTING to live in the time of Jefferson when it was still possible [although not probable] for 1 Individual to master the known base of knowledge.

           Those days are long gone.  

            But we have experts, right?  Yeah, but those experts are just beginning to integrate their limited knowledge bases.

              I got my informational e-mail from "Neurolaw News" -- & what struck me was both how amazing it was and how incomplete it was.

              "Neuroscience" alone is NOT going to get us to a complete understanding of human bad behavior.

               We're IN THE PROCESS of INTEGRATING & UNDERSTANDING the relationships between:
  • Physical & Mental Health
  • Beginning to understand the HUGE IMPORTANCE of:
    • The Immune  System
    • The Microbiome 
    • The Gut
    • The Endocrine System
    • Social Determinants of Health
    • Etc., Etc., Etc.

                So who are we putting the BURDEN on:  poor, marginalized, often traumatized people often with cognitive disabilities.

                  And let's just say a word about that trauma:  sometimes it is from child abuse or war BUT sometimes it is from a SOCIETY that is still finding its way.

                  I don't know how many times I've listened to Judges lecture criminal defendants & thought:  "What a frickin' waste of time."

                 Of course, the truth is it is a "frickin' waste of time" for us to lecture the Criminal Justice System -- They need a lot more support to make the fundamental, structural changes that are necessary just as the people do who are coming before them.


                 Our new theme:  SUPPORT.   We are taking as our first illustrative symbol: THE GREEK COLUMN -- a classic symbol of:
  • WESTERN CIVILIZATION,
  • MARVEL of ENGINEERING & BEAUTY
  • STRONG ENOUGH TO PROVIDE SHELTER TO FUTURE GENERATIONS ​

In one of the biggest breakthroughs in schizophrenia research in recent times, Professor Cynthia Shannon Weickert from Neuroscience Research Australia (NeuRA) and UNSW Sydney has identified immune cells in greater amounts in the brains of some people with schizophrenia.

NO GOOD DEED GOES UNPUNISHED

There's a lot of "no good deed goes unpunished" in our attacks on the Mental Health Profession.

But we are in an intolerable situation with gross knowledge gaps, a scientifically invalid diagnostic manual, treatment stagnation, and horrific moral and ethical problems involving people with mental health needs in the Criminal Justice System, homeless, and in the society at large.

We don't think the Mental Health Profession is going to go away anytime soon, but we do think mental health concerns are going to involve other areas of medicine that may ironically predominate over traditional mental health care such as immunology and there are others.

The Immune System
  • There are probably an infinite number of ways to break an arm
  • It's turning out that there are a whole lot of ways to damage the human immune system and ultimately the brain

​We don't really think some mental health professional relationships are all that healthy ---- We think @ least some of us would like to move to a more traditional doctor patient relationship --- BUT that's been really difficult with the knowledge gaps in the profession.

We're in the midst of a scientific revolution -- Our moral duties don't start after it's ended -- They're here right now

          The Scientific Revolution we're currently in the midst of is going right to the heart of:
  • The Mental Health Profession
  • The Legal Profession, especially the Criminal Justice System, and
  • Our Widespread Ideas of Bad Human Acts + "Intent" as a Justification for PUNISHMENT, 
  • How we provide for Safety.
  • And what treatments are most efficacious for people with invisible cognitive disabilities such as:
    • ​Mental Illness
    • Brain Injury
    • Developmental Disability 
    • Substance Use Issues
    • Etc.

            This is causing not only enormous conflict in the society BUT also enormous internal conflict as we wrestle with these issues and anticipate what may be to us unacceptable outcomes when our systems haven't caught up with the often horrific conundrums we're facing.

                  For example -- if the State was providing adequate bed space and intensive services  -- would we really be struggling as hard as we are with getting people with invisible cognitive disabilities out of the Criminal Justice System.

                 We think that it was that internal conflict and anticipation of undesired consequences that led media commentator Amy Holmes to observe both that convicted criminal defendant Wanda Barzee was:
  • A "psychotic menace," and
  • "We don't know if Wanda Barzee has a mental illness or not."

                       But it is even more complicated than that:  There's plenty of evidence that the Structure of our Criminal Justice System is FUNDAMENTALLY UNFAIR.

                         And the current scientific revolution is not only cutting to the heart of the significance of human "intent" --- it's taking some of those SOFT concerns about social environments and turning them into some pretty HARD biomarkers.

                                Two related examples are how childhood trauma and post traumatic stress disorder can affect the immune system.

                                Well, that's really significant because damage to the immune system can really affect the brain.


                               Now -- are we all the way there yet to clear biomarkers on ALL human behavior -- no, but we are on our way.

                             GASP!

                           In the meantime, we know enough to know:
  • We don't know it all,
  • ​We don't know enough to be making the assumptions we're making in the Criminal Justice System,
  • We have to have safety, and
  • We should be treating all people as humanely as possible.

Let's Do That -- and we're NOT DOING IT NOW.

What we're doing now in the Criminal Justice System is immoral and unethical.

Mental Health is going to have to change a whole Lot to be effective -- & a big part of that is appreciating the large role of the immune system in Mental & brain disorders

We're almost always in the position of not knowing everything.

How we deal with that has ENORMOUS MORAL & ETHICAL CONSEQUENCES for ourselves and others.

We've got a Mental Health Profession that:
  • Doesn't have a valid diagnostic manual; 
  • Many treatments have stagnated or stalled;
  • They have a very incomplete understanding of "mental illness"
  • States & the Federal Government are paying Millions, Billions of Dollars for mental health treatment with pretty underwhelming results;
  • And yet this same profession is going into Criminal Court & testifying in criminal cases often greatly affecting the lives of very poor, traumatized  disenfranchised people.

The practice of using Mental Health Evaluations as a predicate for punishment in some criminal cases has to stop, and really needs to stop TODAY on the part of both State and Private Mental Health Evaluators.

Does that cause problems well yes it does -- because we also have to reform our Criminal Justice System.  Is that going to be hard -- you bet it is.

BUT LIVING THE LIE WE'RE CURRENTLY LIVING IS A SIN.

BBC health reporter James Gallagher explores the increasing body of evidence that a dysfunctional immune system is responsible for the depression or psychotic illness experienced by hundreds of thousands, perhaps millions, of people in the UK.

James talks to the psychiatrists investigating this new understanding of mental illness and to people who may benefit from treatments aimed at the immune systems rather than their brain cells.

"I believe this is one of the strongest discoveries in psychiatry in the last twenty years", says Professor Carmine Pariante of his and other research on the immune system and depression. "It allows us to understand depression no longer as just a disorder of the mind and not even a disorder of the brain, but a disorder of the whole body.

It shifts conceptually what we understand about depression." James also talks to New York journalist Susannah Cahalan.

he began to experience paranoid delusions and florid hallucinations when her immune system made damaging antibodies against part of the molecular circuitry in her brain.

Treatment to eliminate the antibodies prevented her committal to psychiatric hospital.

Psychiatrist Professor Belinda Lennox at the University of Oxford says she has evidence that a significant proportion of people presenting for the first time with psychotic symptoms are victims of a similar autoimmune problem.

The condition causes NMDA receptors to stop functioning properly and can result in symptoms commonly associated with neuropsychiatric disorders.

         Our society is in such DENIAL about mental ill health and brain injury among men in the Criminal Justice System.
          Further, is it an accident that so many women in the Criminal Justice System have histories of sexual abuse, brain injury or mental illness -- probably not.

         Our Society is in such DENIAL about the reality of the people who are finding themselves in the Criminal Justice System, because to acknowledge REALITY would totally upend our Criminal Justice System.
                BUT we can still have SAFETY & EVEN HIGHER LEVELS OF SAFETY IF WE ACKNOWLEDGE REALITY.
                While there is an ENORMOUS amount of EVIDENCE that we don't have "mental illness" or human behavior completely figured out, there is a growing body of SCIENTIFIC EVIDENCE that ALL KINDS OF FACTORS affect HUMAN BEHAVIOR and can DAMAGE the IMMUNE SYSTEM leading to impaired cognition and behavior.
                Let's concentrate on those factors we are aware of -- & that includes THE SOCIAL DETERMINANTS OF HEALTH & the GUT MICROBIOME.

      Alcohol, Dementia and Mental Illness:  It's Pretty Complicated & Plenty of Traps for the Unwary

                I grew up in a somewhat extended family situation.  My paternal grandmother had taught Latin and French and even in her 80's she was very sharp and read her Latin Bible and listened to French Canadian radio.
                      One of the things she did every night before bed was have a glass of Port and some Velveeta cheese. 
                        My grandmother was a very slim person, so she wasn't particularly concerned about her weight.
                         She had a very serious case of osteoporosis -- at the time of her death when she was 88 -- she was totally bent over.   I don't think they realized at that time that alcohol was not a good thing if one had osteoporosis.

                           Well, my Dad didn't really drink or very, very occassionally.   He wasn't morally opposed to it -- but he was always watching his weight and he didn't really like it that much.   As a child and young man, he had been pretty heavy and worked very hard to lose that weight.
                                 He didn't really eat fish, and he specifically didn't like Salmon.
                                   In high school, he had contracted 
      mononucleosis -- and that has been linked to some adult chronic illnesses.

                                Well @ age 59 my Dad was diagnosed with Parkinson's and that would later change to Lewy Body Dementia.

                                    Now none us really believed the problem with Dad was he didn't keep his mind active.  He was a very smart guy and he was still beating us in Trivial Pursuit well into dementia.

                                            It took a long time for the Dementia to destroy his brain, but it finally did. 


       
                                   
       There are so MANY FACTORS to individual disease processes-- some we think we know and some we don't.

                                    Well, more than a decade ago this stuff started coming out about how alcohol might be protective against Dementia.

                                               "If Dad had just had a glass of wine every night like Grandma Corzine -- this may never have happened,"  I thought.

                                             So what did I do?  Well, I started to have a glass of wine every night @ dinner which often included salmon and spinach and walnut salads.

                                                 So they are saying women should only have a glass of wine a day -- BUT I'm like a really short person and I've concluded that's excessive for me.  AND now I have osteoporosis -- I just need to avoid it.

                                                 Well, right after dinner I would pretty much have to go to bed because of the alcohol -- and then I would get up later -- so it totally disrupted my sleep schedule.


                                                 At the time, I didn't know I was susceptible  to Bipolar Disorder or a disrupted sleep schedule could put me over the edge.

                                                      Then I traveled across some time zones and all the sudden I'm having my first and so far only psychotic episode in an Airport in Washington, D.C.

                                                     Of course, I do think there were other factors than the wine and how those factors shake out in any individual person is pretty complicated.

                                                       I've talked a lot about recognizing Altitude as a factor.  It is important to realize that Altitude is a factor primarily for people who are already vulnerable to certain issues, serving as a kind of magnifying effect. 

                                                     So needless to say I am very wary of the health benefits claimed by alcohol.  In women it can increase the risk of breast cancer.

                Like a lot people with mental health concerns, I also have more recognized auto-immune concerns, specifically thyroid concerns.
                 I was interested to see in the list of articles that "osteoporosis" is now being considered for possible inclusion as as an auto-immune disease.
                   Like a lot of older, short women -- I have issues with that, too.
                     So if one has had the experience of one's cognitive functions essentially breaking down -- and I hope you never do -- it's clear to those around you and ultimately to yourself that something is going on -- but what that something is -- isn't very clear.
                          So we just give it our best guess.  Our guesses have been getting better -- BUT still . . .  they can miss the mark rather dramatically.
                           AND that is really the spark or the raging wildfire for the anti-psychiatry movement.
                            I really do believe in science.  AND I think the answer for inadequate science is better science -- and listening to people.
                             Our mental health system costs a helluva lot for a lot of reasons, one of those is that it is just not based on the lastest science or the best treatments for what is essentially in many cases an immune disorder.

      One promising treatment for depression on the horizon is the use of electrical stimulation to change the signals between the brain and the immune system.

      Prof Kevin Tracey, President and CEO, of the US Feinstein Institute for Medical Research, discovered that the brain controls production of a deadly inflammatory chemical called TNF, which if released in high doses can be fatal, causing people to, literally, die of shock.

      He has recently developed a electrical device which reproduces the connection and switches off the chemical. Three quarters of patients with rheumatoid arthritis recovered following trials.

      “This is the tip of the iceberg of a new field called bio-electric medicine,” he said.

      “This is a new way of thinking about medicine. We’re using electrons to replace drugs. This will not replace all drugs. But there will be many drugs that are either too expensive, too toxic which may be replaced by these devices.”​
